These Terms of Service (“Terms”) govern your use of Guardian RN services and website.
1. Services
Guardian RN provides independent nursing oversight services, including but not limited to:
Unannounced visits
Medication reconciliation
Health and wellness assessments
We act as an independent third-party advocate and do not replace facility staff or medical providers.
2. No Medical Advice / Emergency Services
Our services are not a substitute for emergency medical care or physician services.
In an emergency, call 911 immediately
We do not provide diagnosis or treatment plans
3. Client Responsibility
Clients agree to:
Provide accurate and complete information
Authorize access to relevant care environments (facility or home care)
Understand the scope of services provided
4. Communication & SMS Consent
By contacting Guardian RN, you consent to receive communications including SMS related to your inquiry or services.

Reply STOP to opt out
5. Payment & Services
All services are provided under agreed pricing terms.
Failure to pay may result in suspension or termination of services.
6. Limitation of Liability
Guardian RN is not liable for:
Actions or omissions of third-party care providers
Outcomes related to medical conditions
Indirect, incidental, or consequential damages
Our role is observational and advisory in nature.
7. Privacy
Your use of our services is also governed by our Privacy Policy.
8. Termination
We reserve the right to refuse or terminate services at our discretion, including for misuse or non-payment.
9. Changes to Terms
We may update these Terms at any time. Continued use of services constitutes acceptance of updated Terms.
